The Comprehensive Care for Chronic Conditions Program aims to support initiatives focused on primary prevention, expand access to health care and promote screening and continuous monitoring of risk factors in Primary Health Care (PHC).

The aim is to provide comprehensive care, involving a proactive approach to the early identification of chronic conditions and timely intervention, as well as measures such as health education, encouragement of supported self-care and the active engagement of health professionals in establishing continuous and scaled monitoring strategies to ensure the effectiveness of treatment.

By implementing these measures, the aim is to improve health outcomes, reduce complications and ensure integrated care for patients with chronic conditions.
