Approximately 470,000 residents, including riverside, indigenous, and isolated communities in the Lower Amazon and Tapajós regions, in the state of Pará.
Conducted by the Support Center for the School of Public Health of the University of São Paulo (CEAP/FSP-USP), the initiative operates in the Lower Amazon and Tapajós regions in western Pará to strengthen Primary Health Care (PHC) as the coordinator of care and the integration between different care levels. The project involves 12 Basic Health Units (UBSs) across six municipalities: Santarém, Prainha, Almeirim, Rurópolis, Trairão, and Jacareacanga.
With a focus on child reproductive care, oral health, hypertension, and diabetes lines of care, the initiative incorporates digital technologies, telehealth, and self-care strategies adapted to the territory’s reality. Actions include training for Community Health Agents (ACS) and PHC professionals, alongside the delivery of equipment such as blood pressure monitors, fetal dopplers, measuring tapes for maternal uterine height, and digital fundus cameras.
In 2025, the project advanced the development of the Tecendo Saúde (Weaving Health) application, initially focused on the chronic diseases line of care, with user testing scheduled for 2026. Another milestone was the recognition, by the Ministry of Health, of the Telehealth Center of the Federal University of Western Pará (Ufopa) as a public health establishment. The center, established as an outgrowth of the project, already operates in analyzing and issuing exam reports for tests performed with the digital fundus camera at the participating UBSs.
Improved control of hypertension and diabetes, expanded access to oral health services, strengthened prenatal care, reduced prevalence of malnutrition, and full adoption of digital solutions.
