Focused on monitoring hypertension and diabetes, the initiative provides a digital platform with specific modules for Community Health Workers (CHWs) and health managers, as well as an app for end users. The data will be integrated into e-SUS APS and the Primary Care Health Information System (Sisab), enabling real-time tracking of test results, consultations and medications for the populations served

100% of the population over 18 registered, improving the monitoring of hypertension and diabetes care pathways, providing the technological solution as a legacy to municipalities, and generating scientific evidence.
