The Planificação da Atenção à Saúde (Health Care Planning initiative) aims to strengthen Primary Health Care (PHC) by organizing Regionalized Health Networks. Led by the National Council of Health Secretaries (Conass) since 2005, the initiative is supported by the Pan American Health Organization (PAHO), Umane (since 2022), and the National Council of Municipal Health Secretariats (Conasems).

The proposal unfolds into two complementary fronts.

The first works directly on organizing services and work macroprocesses in both Primary Health Care (PHC) and Specialized Ambulatory Care (AAE). It involves coordination with managers, training of professionals, structuring and revising processes, care pathways, and integrating health services in a given region — always based on the population’s needs. The process takes place in stages and sub-stages, advancing as plans are agreed upon with local teams. The expectation is to operate in up to six states, integrated with existing initiatives.

The second front focuses on strengthening the institutional capacity of PHC through the Primary Health Care Leaders (LIS) project, implemented by Vetor Brasil. The initiative selects, trains, and connects a network of twenty public managers — ten public management trainees and ten mid-level managers who receive mentoring focused on leadership and management competencies.

Together, the two fronts form a planning and management strategy that supports a more coordinated, efficient, and responsive model of care.

Structuring of municipal Health Care Networks, strengthening teams, promoting effective service integration, and ensuring appropriate care for the population.
